THE HARVARD PILGRIM HEALTH CARE FOUNDATION supports Harvard Pilgrim’s mission by providing the tools, training and leadership to help build healthy communities throughout Connecticut, Maine, Massachusetts and New Hampshire. Its focus is on increasing access to fresh, affordable food to low- and moderate-income families and communities in the region; supporting healthy, strong communities through grant contributions and community service; and addressing health disparities affecting diverse populations.

THE HARVARD PILGRIM HEALTH CARE INSTITUTE’S DEPARTMENT OF POPULATION MEDICINE (DPM) is a unique collaboration between Harvard Pilgrim and Harvard Medical School. It’s the nation’s only medical school appointing department based in a health plan. The Institute’s mission is to improve health care delivery and population health through innovative research and teaching. Its staff includes leading physicians, researchers, clinicians and public health professionals.

Through a range of collaborative relationships with health plans and health delivery systems, the DPM faculty designs, implements and analyzes novel approaches to solving some of the nation’s most pressing problems of treatment of disease, prevention and public health, with a focus on activities that lead to significant, concrete change.

Areas of focus include: delivery systems-based research, surveillance and public health research, comparative effectiveness and prevention research, and policy research. In addition, the DPM sponsors teaching programs for medical students and residents that provide training in the types of settings where they will eventually practice. These programs instill both clinical skills and a sound knowledge of public and population health issues, including innovations in science and medicine, health care insurance systems, and disease prevention.

TRESTLETREE, a Harvard Pilgrim subsidiary, is a health transformation company that offers a full continuum of health behavior change services focused on the individual, family, workplace, physician practice and community.

TrestleTree’s training and unique behavior change model have achieved measurable and sustainable change for employees and their families, regardless of motivation level or severity of condition, which has resulted in improved health and lower health care costs for employers. TrestleTree’s core expertise of effecting behavior change for the non-adherent individual is also being leveraged within communities that are pursuing a value-based health model. TrestleTree is accredited by URAC for disease management.
